Office Procedure

• Please arrive 15mins early for your scheduled appointment

• When you arrive by car you can go through our back entrance (off Executive Ct.) or our main entrance (Off Corporate Hill Dr.). If you drive through our back entrance please be aware that you will need to circle around our building to access the main entrance to get inside as our back access into the building is for employees only.



• If you are coming for a well visit, please feel free to come inside to our Well Clinic

• If you are coming for a same day or sick visit, please feel free to come inside to our Acute Care Clinic

• We see all potentially infectious patients in our Acute Care Clinic. Which is separate from our well clinic to limit exposure.

• Upon appointment check in we will ask that we take a copy of your insurance card and photo I.D. 

• Patient Registration forms will be required to out fill once a year during check in. 

• We only see walk-ins 8am-9:30am Monday-Friday for ages 6 months+ (see what is accepted as a walk-in under "servies" & "sick visits"). Same day sick appointments are only scheduled day of, first come first serve. Our phones are on as early as 7:45.
